The following is a collection of frequently used WMBA forms for coaches, which can be downloaded, printed and submitted as required. Files are provided in the Adobe Acrobat '.PDF' format. To view either one of the file formats, you must have Adobe Acrobat Reader installed on your computer.
The contents of all the files are the property of the WMBA and are to be used exclusively for WMBA programs only. All forms can be printed on a standard 8.5" X 11" piece of paper unless otherwise noted.

Please EMAIL the WMBA your name, club name and mailing address and we will mail you an original form to complete. These forms must be submitted in person to the on-site Gym Supervisor at the start of the season. Forms can also be obtained from your basketball convenor. Form used by the WMBA to screen its coaches through the MB Child Abuse Registry Program. For use by the Head Coach, Assistant Coach, Team Managers and Club Convenors only!
A form used by coaches at the Premier or Rising Stars level to evaluate players at identification camps. For use by WMBA Coaches only.

Official Game Scoresheet (8.5" X 14" paper required)
Official game scoresheet used in all WMBA sanctioned games.
If printing to legal-size paper is not an option, you can print the PDF file on ordinary letter-size paper (8.5 x 11"). Follow the procedure you normally use to print PDF files to letter-size paper. When the Print window appears on your screen, select the 'Shrink oversized pages to paper size' or 'Fit to page' or 'Shrink to Fit' option. Continue with your normal printing procedure.
Form used by clubs or teams looking to host their own basketball tournament. Sanctioning provides insurance coverage and support of the WMBA in hosting the event.
